Proficient Rehab is proud to contract with several area home health agencies to provide exceptional physical therapy to all of Southeast Texas, whether in our clinic or in your home. If you are already receiving home health care and are needing physical therapy, ask your case manager if Proficient Rehab is an option for you. From post-surgery, acute illness, to new physical setbacks, we are ready to work with you to return to doing the things you love.
